CENTER POLICIES
APPOINTMENT POLICIES
Reservation Requirement: To reserve an appointment, ClarityMD requires a credit card. New patients must provide this information at least 48 hours before their first appointment, or the appointment will be canceled. Arrival Time: Please arrive 10 minutes before your appointment to complete the check-in process and prepare for treatment. Arriving late will shorten your scheduled service time. Patients arriving 15 or more minutes late will be rescheduled and charged a $50 non-refundable fee. Appointment Confirmations: We offer appointment confirmations via email, text, or phone call, but these are not guaranteed. It is the patient’s responsibility to keep track of their scheduled appointments.
CANCELLATIONS AND NO-SHOWS
Notice Requirement: We require 24-hours notice for rescheduling or canceling appointments. Failure to provide adequate notice will result in non-refundable fees. The first missed appointment without adequate notice is considered a grace offense. A second offense will incur a $50 non-refundable fee. On the third offense, you will be charged the full amount of the scheduled service. This policy helps us compensate our employees and increase availability for other patients.
RIGHT TO REFUSE
ClarityMD maintains the right to refuse service to anyone exhibiting inappropriate or disruptive behavior.
MINORS
We respectfully request that children under 18 do not accompany you into the treatment room for your scheduled service. Lasers and other flashlamp treatments pose a potential hazard to eyesight. Minors may not be left unaccompanied in lobby or waiting areas.
PRODUCTS & SERVICE RETURNS
All product and service sales are final. Unless otherwise stated, prepaid treatments and packages expire one year from the date of purchase.
PAYMENT METHODS
We gladly accept cash, all major credit cards, FSA/HSA cards and ClarityMD gift cards. CareCredit is available for purchases of $1,000 or more.
